If You Receive Social Security Benefits:
If you are receiving benefits from Social Security and the Railroad Retirement system, you will automatically receive Medicare coverage for Parts A and B. The coverage starts on the first day of your birthday month.
If You Don’t Have Social Security:
If you don’t receive Social Security and Railroad Retirement system benefits, you are eligible to start your Medicare application three months before and after your 65th birthday.

If you’re 65 and older, still employed, and covered by group health insurance, however, you’ll need to follow a different set of steps.
A majority of group health plans require you to sign up to Medicare, so it’s best to check with your benefits department first. Your group plan becomes secondary at this point, with Medicare paying first. Unless your benefits department tells you otherwise, it’s best to apply for Medicare even if you are still working.
If you’re over 65, you’ll have a Special Enrollment period when you can add Part B to your group health plan benefits. This can happen if you terminate employment before reaching 65.
